The Universal referral form for scuba is an online tool which can be tailored to specific needs. You can add an electronic signature to it and even print out the completed form, as well as share and save it. If you have any questions, you can contact support to get assistance. Contact the support team if you have any questions or concerns about the form. They will be happy to help you.

PADI Open Water Diver Certification

A student diver can present the PADI Universal referral for Open Water Diver certification to a instructor. It states that a diver had initial training from another diving organization. The form directs the instructor of diving to complete the student’s open-water dives and sign a declaration. This statement should be sent to the original instructor so that they can certify the diver.

PADI will require that you complete the course within 12months of your last training. This usually takes two to three days. You and your buddy must be able, at all times, to dive to a maximum depth not less than 60 feet/18 m.

Medical statement

To become a certified scuba diver, you will need to complete a Medical Statement for Universal Referral. This document is vital and will make sure you are as healthy as you can before you dive. It's designed to ensure your safety while you're underwater, so make sure to answer every question honestly.

First, make sure your physician signs the form. This is a requirement when you first start training as a scuba diver. If you have a new condition, a medical release will be required. A photocopy of your certification card is recommended.

Logbook

A medical statement is required before a diver can open-water dive. Generally, this will be done before a certification course begins. A physician must sign the medical release. You will need a new release if your medical situation changes. PADI offers a medical release. The medical release is equivalent to the PADI Release of Liability/Assumption of Risk/Non-agency Acknowledgement Form - General Training or the EU version.

The universal referral forms are different from those used in a dive logbook. The student will be able to list all open water dives completed and the knowledge development sections. It also contains a signed statement from the instructor confirming that the diver has taken the required dives. The student must then take this form to the instructor, who will certify him/her as a certified diver.

You must complete the course

Most well-known scuba certification agencies participate in the Universal Referral Program. This means that a diver could take the theory, pool and open water courses with the local dive center while completing the open water inspection dives with another agency. This can be done by a dive center that has a universal referral program like SSI or NAUI, NASDSS, YMCA, and other organizations.

PADI Open Water Diver courses require that the student passes the Knowledge Development and Open Water Dives sections. Before diving, the student must sign a medical release from their physician. The student diver must also provide a photo to accompany his or her certification card. Digital images can be accepted.
